At Appointed Seasons Counseling and Coaching, we are committed to protecting your privacy and the confidentiality of your personal and health information. This Notice applies to all records of your care created or maintained by our practice, whether generated by counselors, coaches, staff, or business partners who support our services.

We may use and share your health information with other professionals involved in your care to provide, coordinate, or manage counseling and coaching services.

We may use and disclose information to bill and receive payment for the services we provide.

Right to Request Restrictions – You may ask us not to use or share certain information for treatment, payment, or operations (though we are not required to agree to all requests).
Right to Confidential Communications – You may request to receive communications in a specific way (e.g., at a certain phone number or address).
Right to a Paper Copy – You may request a paper copy of this Notice at any time.

Our Responsibilities

We are required by law to:

Maintain the privacy of your protected health information.
