This Notice of Privacy Practices (“Notice”) describes how your protected health information (PHI) may be used and disclosed, and how you can access your information. Please review it carefully.

Our Commitment to Your Privacy

At Appointed Seasons Counseling and Coaching, we are committed to protecting your privacy and the confidentiality of your personal and health information. This Notice applies to all records of your care created or maintained by our practice, whether generated by counselors, coaches, staff, or business partners who support our services.

How We May Use and Disclose Your Information

We may use or share your information in the following ways:

 

For Treatment

We may use and share your health information with other professionals involved in your care to provide, coordinate, or manage counseling and coaching services.

 

For Payment

We may use and disclose information to bill and receive payment for the services we provide.

 

For Health Care Operations

We may use information to run our practice, improve services, conduct quality assessments, and train staff.

 

Other Uses and Disclosures Permitted or Required by Law

We may disclose information in the following circumstances without your written permission:

  • When required by law (e.g., reporting abuse, responding to legal proceedings).

  • To prevent serious threats to health or safety of you or others.

  • For public health and safety activities, such as reporting communicable diseases.

  • For health oversight activities by government agencies.

  • To comply with workers’ compensation laws.

 

Uses and Disclosures Requiring Your Authorization

Any other use or disclosure of your PHI will be made only with your written authorization. You may revoke an authorization at any time in writing, except to the extent we have already acted in reliance on it.

 

Your Rights Regarding Your Health Information

You have the following rights under HIPAA and applicable law:

  • Right to Access – You may inspect and request a copy of your health records.

  • Right to Amend – You may request corrections to your records if you believe they are incorrect or incomplete.

  • Right to an Accounting of Disclosures – You may request a list of certain disclosures of your information.

  • Right to Request Restrictions – You may ask us not to use or share certain information for treatment, payment, or operations (though we are not required to agree to all requests).

  • Right to Confidential Communications – You may request to receive communications in a specific way (e.g., at a certain phone number or address).

  • Right to a Paper Copy – You may request a paper copy of this Notice at any time.

 

Our Responsibilities

We are required by law to:

  • Maintain the privacy of your protected health information.

  • Provide you with this Notice describing our legal duties and privacy practices.

  • Notify you if a breach occurs that compromises the privacy or security of your information.

  • Abide by the terms of this Notice currently in effect.

We reserve the right to change this Notice at any time. Updated versions will be available on our website and upon request.
